IMPORTANT NOTE: Flu Shots MUST be PRE-REGISTERED for to assure availability! Registration for shot must be submitted INDIVIDUALLY for yourself and/or EACH Family Member!
Flu Shots are FREE for adults aged 18 and over when you show your driver’s license with either your Employee ID Card or Insurance Card.
Flu shot Registration: To reserve your FLU shot with the VNA Clinic or go to https://flu.vna.org/ . Use the codes below to register, and print consent form using link below.
